- Admission is limited. EU, Italian, and equivalent applicants are ranked through an admission test; non-EU applicants residing abroad and requiring a visa are assessed on their academic and personal background and, after positive preliminary evaluation, on an interview. Applicants must hold a first- or second-cycle university degree, or an equivalent foreign qualification recognized by the University, and must have at least 50 ECTS in the biomedical/scientific areas listed by UniMi for the programme (including physiology, biochemistry, molecular biology, clinical biochemistry, biology, pharmacology, human anatomy, genetics, microbiology, medical statistics, human genetics, pathology, oncology, internal medicine, mental health, neurology, laboratory medicine, informatics, and medical physics). Applicants from Italian or EU universities must have 180 ECTS; graduating students may apply if they have at least 156 ECTS by the application deadline and complete the degree by 31 December 2026. For the entrance-test route, applicants must score at least 20/50 to be included in the ranking list.

SLAM verifies this through one of three routes: a recognized B2+ certificate issued no more than three years before the application date and uploaded with the application; a University of Milan SLAM-certified English level achieved within the previous four years (automatic verification, no upload needed); or the SLAM placement/entry test. Applicants who fail or miss the SLAM test must submit a recognized certificate via InformaStudenti by the programme deadline shown on the UniMi language page for this course, which is 31 December 2026, or they will not be admitted. The course-specific call also states that admitted non-EU visa applicants can enrol only if they pass the Italian language proficiency test, except where exempt, but it does not specify the CEFR level; this conflicts with the generic UniMi international enrolment page, which limits that Italian test to degree programmes taught in Italian.

- EU, Italian, and equivalent candidates sit a competitive written admission test in English on 10 September 2026, delivered remotely according to the call. The test has 50 multiple-choice questions in biochemistry, biology, pharmacology, physics, genetics, immunology, and pathology. Scoring is +1 for each correct answer, 0 for unanswered questions, and -0.5 for each wrong answer. A minimum score of 20/50 is required to enter the ranking list. Tie-breakers are: higher number of correct answers, then younger age. Non-EU candidates residing abroad and requiring a visa do not sit this written exam; they are selected through evaluation of their scholastic/personal curriculum and, if positively pre-screened, an interview on subjects related to the required scientific-disciplinary areas.
