An Italian secondary school diploma or a foreign qualification certified as equivalent is required. Applicants must also demonstrate knowledge of the cultural prerequisites (Biology, Chemistry, Physics) defined by the annual Ministerial Decree governing the admission test; if the IMAT result shows deficiencies (score below 25% of the maximum in Biology, Chemistry or Physics for any discipline, or below the overall minimum threshold), the Degree Programme assigns Additional Learning Requirements (ALRs) to be satisfied in the first year. Students who are not native Italian speakers and lack basic Italian at admission must attend Italian-language courses (offered by CLAM or equivalent) to reach at least B1 CEFR during the course, needed for patient-facing clinical activities.

The university-wide international.unime.it admission page explicitly states: 'For the Single-Cycle Degree Programme in Medicine and Surgery (taught in English): the English Language Certificate is not mandatory.' It also lists standardised English-language admission tests such as the IMAT as grounds for exemption from the International English Certificate 'only if the test is a compulsory or preferential requirement for admission to the chosen degree programme.' However, the programme's own admission-requirements page (medicine-and-surgery.cdl.unime.it) states English knowledge must be certified at minimum CEFR B2 for non-native speakers, though it notes 'the scientific and technical terms used in the course access test (IMAT) are considered higher than the B2 level of knowledge required,' implying that passing the IMAT itself is treated as satisfying/exceeding the B2 requirement rather than requiring a separate certificate. Net practical reading: no separate English certificate is required as long as the applicant sits and passes IMAT (in English), consistent with the 'not mandatory' framing on the international admissions page, but this could not be fully reconciled between the two official sources.

Admission is via IMAT (International Medical Admission Test) — a mandatory national entrance examination for all candidates, including international applicants, organised by the Italian Ministry of University and Research (MUR) under Law n. 264 of 2 August 1999 and subsequent amendments. Number of places, test date, content and selection procedure are set annually by MUR decree and published in the annual admission call. For the 2026/2027 cycle, UNIME's 5th Call - Medicine and Surgery (IMAT), open to all candidate groups, runs from 20 June 2026 (00:01 CEST) to 20 July 2026 (23:59 CEST), 'to be confirmed based on publication of the relevant Ministerial Decree'. An IMAT 2025/2026 information page referenced a test date of 17 September 2025 for the prior cycle; the exact 2026/2027 IMAT date was not found on official pages at the time of this research.
